Primary Care Networks (PCNs) are clusters of local GP Practices working together to improve the health of their local community. PCNs are a key part of the NHS Long Term Plan, with all general practices being required to be in a network and the local Integrated Care System required to support their work.
The networks have expanded neighbourhood teams which are made up of a range of staff such as GPs, pharmacists, district nurses, community geriatricians, dementia workers and Allied Health Professionals such as physiotherapists and podiatrists/chiropodists, joined by social care and the voluntary sector’.
All of the GP Practices in Lewisham are a member of a PCN. Our PCN covers Downham, Catford and other parts of south Lewisham.
We are seeking to work better with other local organisations such as the council, hospital, community Services, and the voluntary sector. In order to do so we will be crating an Integrated Neighbourhood Team so that everyone working on the health and wellbeing of our residents is doing so in a joined up way that makes a real difference.
