About Us

Sevenfields Primary Care Network is one of several Primary Care Networks (PCNs for short) in Lewisham. Working in partnership with the community we serve, we transform primary care in a sustainable way that positively impacts on the health and wellbeing of the local population.

Primary Care Networks (PCNs) are groups of local GP Practices working together to improve the health of their local community. PCNs are a key part of the NHS Long Term Plan, with all general practices being required to be in a network.

The networks have expanded neighbourhood teams which are made up of a range of staff such as GPs, pharmacists, district nurses, social prescribers, health & wellbeing coaches, care navigators and more. You can find out about some of the services we offer here.

All of the GP Practices in Lewisham are a member of a PCN. Our PCN covers Downham, Catford and other parts of south Lewisham. You can click here to view which GP Practices we cover.

We are seeking to work better with other local organisations such as the council, hospital, community Services, and the voluntary sector. In order to do so we will be creating an Integrated Neighbourhood Team so that everyone working on the health and wellbeing of our residents is doing so in a joined up way that makes a real difference.
