Projects

We have three major projects underway at the moment. These are:

Health Inequalities

We know that our residents in Downham, Catford and elsewhere have poorer health outcomes than we would wish. We are undertaking targeted work with the worst affected communities in our area to tackle this. Our team working on this project includes GPs, Nurses, Care Coordinators, Social Prescribers  and Health Coaches as well as a number of local voluntary sector partners such as the Blue Ribbon Foundation, Goldsmiths Community Centre and many more.

Diabetes project

We are seeking to help our patients manage their diabetes better and to support our residents to make lifestyle changes to avoid the risk of having diabetes in the first place. Our specialist team comprises GPs, Diabetic and other Nurses, Nurse Associates, Care Coordinators, Social Prescribers  and Health Coaches.

Anticipatory Care

Our latest project is Anticipatory where the focus is on preventative care especially for our older population, our most frail residents and those experiencing mental ill health. We use a multi-disciplinary team approach meaning that our team working on this project includes GPs, Nurses, Care Coordinators, Social Prescribers, Health Coaches, social workers, adult social care workers and voluntary sector organisations like Age UK.
